The Five Whys
The Five Whys is a simple yet powerful technique for uncovering the root cause of a problem by repeatedly asking 'Why?' until the fundamental issue is revealed. It helps to move beyond surface-level symptoms to address the underlying cause.
When to use The Five Whys
Use this method when you need to quickly identify the root cause of a problem, especially when the cause is not immediately obvious. It's useful for troubleshooting, process improvement, and preventing recurrence of issues.
What it solves
Addressing symptoms instead of root causes; recurring problems; inefficient processes.
How to run The Five Whys, step by step
- Define the Problem (2 min). Clearly state the problem you want to investigate.
- Ask 'Why?' (3 min per 'Why?'). Ask 'Why?' the problem occurred. Record the answer.
- Repeat 'Why?' (Repeat 3-5 times). For each answer, ask 'Why?' again. Continue until you reach the root cause.
- Identify the Root Cause (2 min). The final answer is likely the root cause of the problem.
- Develop Solutions (5 min). Brainstorm solutions to address the root cause.
Materials needed
- Whiteboard or virtual whiteboard
- Markers or digital pens
- Timer
Facilitator tips
- Encourage participants to be honest and objective in their answers.
- Avoid blaming individuals; focus on the process or system.
- Stop asking 'Why?' when you reach a fundamental cause that cannot be further broken down.
Common pitfalls
- Stopping too early before reaching the true root cause.
- Blaming individuals instead of identifying systemic issues.
- Getting sidetracked by tangential issues.
Variations
- Use a fishbone diagram (Ishikawa diagram) to visually organize the 'Why?' questions and answers.
- Combine with other root cause analysis techniques like fault tree analysis.
Running it online or hybrid
Use a shared document or whiteboard where everyone can contribute to the 'Why?' chain. Encourage participants to type their responses concisely.
What it produces
A chain of 'Why?' questions and answers leading to the identification of the root cause of the problem.
